Health Screenings, Wellness Promotion and Case Management
CIFHS has partnered with Azusa Pacific University School of Nursing's nurse practitioner program and the Neighborhood Wellness Center (NWC) to provide services to its clients. Nurses and nursing students provide screening and information on chronic diseases such as diabetes, congestive heart failure (CHF), and hypertension; foster health promotion activities such as good nutrition and exercise; and provide case management to clients that result in referrals necessary in obtaining medical treatment and follow-up. CIFHS focuses on preventative care and improving community wellness by actively engaging adults and children in the community.
CIFHS partners with community agencies to offer primary care and outreach health services, including:
